States with "Fill in the blank" space for LIDAR distance

Does anyone which states have this on citations? Also, what states (besides Iowa) will completely refuse the distance to you for use in court?

Just thought it would be an interesting discussion. In Iowa, a Uniform Citation is used, distance does not have to be on the ticket, although it can be.

There have to be some states with a distance requirement on citation.
