When a CHP officer is talking through the handset that he has mounted on his shoulder, is this just a walkie-talkie or a full radio? Can he talk to the dispatcher directly through this handset, to other police cars, or to both?

Both dispatch and other cars, walkie-talkies are kids toys.
The belt mounted radio transmits on and receives from the same frequencies as the radio in the car. The only difference is the antenna really, aside from the obvious things like a size difference and a different make/model... the antenna on the radio in the car will be able to reach a greater distance.
The shoulder microphone is just an add-on to the radio for convenience.
