User error.
(1) Like my other brothers here have said - use the Kill.
(2) Excessive speed/obvious speed (i.e. lack of surrounding buffer traffic). If you're a habitual speeder or "hyper-speeder," sooner or later, you've just gotta cowboy the F-up and pay the piper. You've gotta pay to play.
